Statement of Service

An employee or former employee may request a statement of service as evidence of their employment.

School-based employees on the Department’s central payroll

A school-based employee paid on the Department’s central payroll should submit their request to their current school or to their last school if no longer employed.  The request should include the employee’s name, Employee ID and the address to which the statement should be sent.

The school should forward the request to the appropriate Schools HR Services EduMail regional mailbox.  Schools HR Services will provide the statement direct to the person.

The school has the option of providing the statement of service directly to the employee from eduPay.

Employees on CASES21 local payroll

Where an employee paid on the CASES21 local payroll requires a statement of service, a request should be made to each school at which they have been employed.

The statement provided by the school should contain the following information:

  • full name of person
  • date of birth
  • start and end dates of employment;
  • time-fractions or hours worked per week for each period of service
  • position(s) held for each period of service
  • whether the service was casual, fixed term or ongoing
  • any leave without pay (if none has been taken, the statement must show 'nil leave taken'
  • any long service leave taken
  • any payment in lieu of long service leave

A VPS employee who requires a statement of service should contact Corporate HR Services.
